Sports/Student Athletes
Any student wishing to participate in a sport or use the weight room need to submit the required state physical forms. If the physical was performed 90 days prior to the start of the sport, a health update must also be submitted. The required health office forms are located at the bottom of this webpage. For all other required forms, please go to the Athletics Webpage. Playing one sport does not guarantee eligibility for the next sport. Please see the nurses before the registration deadline to determine what is needed to be cleared.
Required in order to be cleared to participate:
Physical completed by your doctor on the state-mandated forms. Please make sure all forms are filled out completely, signed by athlete and guardian on the first page and they are signed by the doctor and have the doctor's stamp.
Impact test completed for all 9th and 11th graders.
All athletes need to be registered on the rSchool Registration Site
If your physical is over 90 days old please complete the health history update form
If your child has asthma, allergies, or diabetes a health plan will need to be submitted. All forms are listed at the bottom of this page.
Sports Physicals
*IMPORTANT UPDATE*: Due to new sports physical regulations by the state of NJ we will no longer be offering sports physicals twice a month at school. The new regulations highly encourage that all sports physicals be done by the athletes primary physician, as they know the athlete best. If you do not have a primary medical home or have special circumstances please call the nurse’s office and we will assist in making arrangements for your athlete to receive a sports physical.
9th Grade and Transfer Students
Need to submit a recent immunization record from their doctor prior to the first day of school.
Submit a recent physical to the nurse if not playing a Fall sport prior to the end of September.
Medication Administration: Over the Counter and Prescription
The school nurse, according to the West Morris Regional High School District standing orders, may administer designated Over-The-Counter (OTC) medications during school hours. For the students' convenience, we stock Tylenol (Acetaminophen), Advil (Ibuprofen), Benadryl (Diphenhydramine), and Tums (Calcium Carbonate Antacid).
All PRESCRIPTION MEDICATION must be accompanied by written permission from both a parent/guardian and physician. There are separate forms for prescription medication, self-administration of inhalers, and self-administration of epinephrine auto-injectors.
Prescription medication shall be brought to the school by the parent/guardian unless other arrangements have been made with the nurse. It must be in the original prescription container, labeled with the name of the student, medication, dosage and name of the physician. All medication is to be taken home by the parent when it is no longer needed or at the end of the school year.
The West Morris Regional Board of Education forbids any student from carrying any medication (OTC or prescription) without the knowledge and consent of the administration and school nurse. Medication includes all medicines prescribed by a physician including emergency medications, such as inhalers, epinephrine auto-injectors and all non-prescription OTC medication.
